The ASA proposal for membership form shall be initiated online, by an ASA member, via the Members Only Area.

A current CV, reprints of the Candidate's three most important contributions to the surgical literature, and a complete bibliography should be submitted in accordance with the Instructions for Completion of the Proposal for Membership.
