The ASA proposal for membership form should be filled out and signed by the three sponsoring Fellows.

A current CV, reprints of the Candidate's three most important contributions to the surgical literature, and a complete bibliography should be submitted in accordance with the Instructions for completion of The Proposal for Membership.
